Introducing Well and Able’s AI Forms

Well and Able has launched a new AI-powered online forms system designed to make completing and submitting forms faster, easier, and more flexible for clinicians, carers, and families.

The new system allows you to:

• Complete forms online from any device

• Use your mobile phone to upload photos instantly

• Download and complete fillable PDF forms as before

• Upload handwritten or completed forms for processing

• Submit supporting documents, photos, videos, and foot tracings in one place

All existing forms are available through the Online AI Forms portal, and any new forms will be added there as they become available.

One of the biggest improvements is the ability to complete forms directly on your phone while visiting clients. Instead of carrying PDFs or returning to a computer later, you can complete and submit forms on-site, including taking photos directly from your camera during the process.

The system also includes AI-assisted checks to help reduce common errors before submission, such as:

• Missing NDIS numbers

• Incomplete therapist details

• Missing plan manager information

• Incorrect measurement entries

This helps streamline the process for both submitters and the Well and Able team.

While all methods are supported, forms completed online provide the clearest formatting and help our team review information more efficiently, especially when combined with uploaded photos and videos.
